It unlocks the gargoyle spirit shroud for that character, useable whenever and as often as you wish. It can only be claimed once per account, as it notes when you /claim to confirm you really want on that character, and you do lose the tooth on turn in (sort of sad, it's a neat icon, though useless other than for turnin to get the shroud enabled).
