Questions Regarding Accuracy Stat on Items & its Function

I have a few questions regarding accuracy stats on items and I can't find answers for them.

1. Does accuracy increase chances of landing a melee hit or does it increase chances of landing a range attack ? In other words, is it applicable for melee or range attack or both ?
2. Is the accuracy stats expressed as % ? For example if accuracy = 1, does it mean that it is 1% ?
3. I assumed that accuracy from various worn items stacks and if so, is there a limit ?

Accuracy is a mod2 that increases yoru chance of landing a hit in melee.
It stacks up to a cap of 150.

Now as a GUESS you will see 1% more hit rate for every 10 to 15 accuracy.

So if you have a 60% hit rate with 0 accuracy with 150 accuracy you would see between 70 and 75% hit rate.

Actually, how big a gain you get from Accuracy depends on your current hitrate, since Accuracy doesn't directly affect your hitrate - rather, it reduces your miss rate. Or something like that. There's a few parses about it over at eqoutrider - I'll see if I can dig it up.
